When the Texas heat cranks up, nothing hits the spot quite like a frosty, perfectly blended milkshake.
Finding the best milkshakes in San Antonio means working through a delicious mix of classic diner-style malts and over-the-top, sugar-loaded creations. From simple vanilla done right to towering shakes piled high with toppings, there’s no shortage of options.
To narrow things down, we focused on spots known for consistently smooth textures, bold flavors, and treats that actually live up to the hype.
Whether you’re looking for a post-burger dessert or a kid-friendly afternoon pick-me-up, these local favorites deliver on every level.

1. The Shack

Address: 7431 Northwest Loop 410 Ste 115
Contact: 210-375-4097 | [email protected]
Hours: Tuesday-Sunday, 11:00 AM – 9:00 PM
Walking into The Shack feels like finding a neighborhood snack counter that never stopped having fun. The San Antonio-born spot started as a snow cone stand, and that playful energy still shows in its milkshake lineup.
We like that the flavors go past the usual chocolate and vanilla. Abuelita Mexican Chocolate, Horchata, Oreo, and Strawberry-Oreo give the menu a local twist.
The shakes also pair naturally with their burgers, dogs, and frozen treats.
When we stopped by, I went for their Strawberry-Oreo shake. The texture was thick without feeling impossible to sip.
The drawback, though, is the setup. The Northwest Loop 410 location sits inside a Chevron, so it does not have the cozy sit-down feel of a full dessert parlor.
We also did not see clear dairy-free shake options listed.
Their bomb sweet treats
This is one of my favorite burger, dog, and Philly spots in SA. I love their burgers on sourdough bread and my new favorite is the Shackrizzo Dog. This thing was full of flavor. I like to come with the kids when we want to go all out on dinner, meaning burgers, dogs, and their bomb sweet treats. My son’s favorite is the chocolate shake and my other kids love the Oreo Churro Split. Ultimately, you just can’t go wrong here.

The Strawberry Shake tastes so sweet
I ordered Texas Burger and it tastes good! Meaty and delicious! I also tried the Strawberry Shake and it tastes so sweet as I like it! I really like how they put a tissue box outside so you can use that for opening the handle of their door. Thank you for keeping us safe! For sure we will come back to try their other foods!

2. Cheesy Jane’s

Website: https://cheesyjanes.com/
Address: 4200 Broadway
Contact: (210) 826-0800
Hours: Tuesday-Thursday, 11:00 AM – 8:30 PM; Friday-Saturday, 11:00 AM – 9:00 PM; Sunday, 11:00 AM – 8:30 PM
Cheesy Jane’s is the kind of diner that makes a milkshake feel like part of the whole meal, not just an add-on. The Broadway spot leans into the retro look with burgers, fries, malts, and 16-ounce shakes served in the original metal mixing tins.
The big draw is variety. Their menu lists more than 30 shake and malt flavors made with their signature ice cream, so it works whether you want something simple or a more playful dessert.
We also think it’s a great spot for families because the room feels casual and easy.
The tradeoff is that extras add up. Turning a shake into a malt costs more, and larger groups can see automatic gratuity added.
A San Antonio classic that never disappoints
Cheesy Jane’s is a San Antonio classic that never disappoints. The retro diner vibe is fun and welcoming, and the staff always makes you feel at home. Their burgers are the real star, juicy, flavorful, and cooked just right, with plenty of creative toppings. Whether you’re craving a casual lunch, late-night bite, or just a milkshake fix, Cheesy Jane’s delivers that nostalgic comfort food experience every time.

3. Sugar Factory – San Antonio

Website: https://sanantonio.sugarfactory.com/
Address: 849 E Commerce St #109
Contact: (726) 204-2020
Hours: Monday-Thursday, 12:00 PM – 11:00 PM; Friday, 11:00 AM – 12:00 AM; Saturday, 10:00 AM – 12:00 AM; Sunday, 10:00 AM – 11:00 PM
Sometimes you want a milkshake that looks ridiculous in the best possible way, and Sugar Factory delivers exactly that.
The downtown location turns dessert into a photo-ready event with chocolate-covered mugs, candy toppings, and brownies. The colorful presentations also feel made for birthdays.
We like that it combines a full-service restaurant with a retail candy area, so it works as a bigger outing near the River Walk. It also makes sense if you’re already comparing local candy stores for a sweet-themed day.
The drawback is the price and noise. These shakes sit in a luxury dessert tier, often costing much more than a casual diner’s shake.
Peak hours can also mean long waits and a loud dining room, so it is not the best choice for a quiet treat.
The strawberry cheesecake milkshake is a must
The sweets are amazing! I would suggest you skip the rainbow sliders, they’re terrible lol, but you get cute rubber duckies with them. The customer service is phenomenal! The GM Gil took the time to make sure we were taken care of. You can tell this place is managed properly, the environment is super fun and exciting. Oh! And the strawberry cheesecake milkshake is a must!

The glass was covered in frosting and gummies
Took my 7 yr old Granddaughter. She LOVED the “girlie” decor, Princess Sliders with pink buns, and Milkshake. The glass was covered in frosting and gummies. Located in the River Center Mall. Outside covered patio overlooked the Riverwalk.

4. URBAN – Dessert Dealers

Website: https://iwanturban.com
Address: 19178 Blanco Road #106
Contact: (210) 251-2925
Hours: Monday-Sunday, 10:00 AM – 1:00 AM
URBAN – Dessert Dealers is the place we’d recommend if the craving hits after dinner, and everything else feels closed. The Blanco Road shop stays open until 1:00 AM every night, which makes it useful for late-night dessert runs.
Their menu feels more experimental than classic. You will find loaded shakes, cheesecake sticks, fresh mix-ins, and a reusable 17-ounce cup program that gives customers half-off refills.
We tried the refill setup and liked that it made a specialty shake feel less like a one-time splurge.
It’s also a fun stop after nearby Japanese restaurants when you want something sweet but not predictable.
The catch is convenience. There’s only one storefront, and the facility handles common allergens, so sensitive diners need to be careful.
Everything comes in a fun container
This place has such a cool vibe! Walk in and order on a touch screen while your desserts are prepared behind the mysterious wall. What’s happening behind the curtain? Who knows. But it’s delicious. Everything comes in a fun container. If you buy a pro shake you can bring your container back in to get half off of a refill. Definitely coming back!

The vibe is really nice and cozy
Me and my family loved it here! The vibe is really nice and cozy, and the containers are very unique in their design! I loved the milkshakes too! I got a coffee one and it was really tasty.

5. Burger Boy

Website: https://burgerboy.com/
Address: 2323 N. St. Mary’s St
Contact: (210) 735-1955
Hours: Sunday-Thursday, 10:30 AM – 12:00 AM; Friday-Saturday, 10:30 AM – 1:00 AM
Operating since 1995, Burger Boy has the old-school San Antonio charm that makes a shake feel tied to local routine. Its St. Mary’s Street location still feels like the kind of place people hit for burgers, fries, and a cold dessert after work.
The shakes are straightforward but satisfying, with specialty flavors like Big Red and Strawberry Cheesecake.
Personally, I also like the Orange Freeze because it gives you that citrusy slush-style option when a heavy dairy shake feels like too much.
The biggest downside is patience. Drive-thru lines can stretch past 30 minutes during rushes. The original parking lot also feels cramped enough to make traffic spill toward the road.
Indoor seating is limited too, so this is often better as a grab-and-go stop.
The shake was thick and not too sweet
In my opinion one of the top burger spots in San Antonio. Luckily they’re a chain so the results elsewhere should be just as consistent. In terms of value and quality you cannot beat this. The burger was juicy. Fries were crispy and fluffy on the inside. The shake was thick and not too sweet. Could not have asked for a better burger. Will always come back and grab Burger Boy before I leave town.

Their shakes are delicious
Amazing! The food is great, authentic, and leaves you wanting more! I had no issues or concerns, the staff, especially Eddy, was very helpful and super friendly. The environment is a neat spot to chit chat and enjoy some good quality American food! Not to mention their shakes are delicious! Everyone in San Antonio needs to try Burger Boy at least once. 10/10.
