  • Financial Accessibility: Paying for senior care often strains family budgets significantly. We prioritized centers that accept various insurance plans like Medicare and Medicaid to ease out-of-pocket expenses.

Getting comprehensive skilled nursing and rehabilitation tailored to the local community is a major priority for many families, and that’s exactly what Avir at San Antonio offers.

Operating under the Avir Health Group, this center provides a combination of around-the-clock medical attention and therapy.

Their facility handles complex medical requirements for both short-term recovery and long-term stays. Because they are affiliated with a wider Texas network, they maintain consistent operational standards that give relatives peace of mind.

What’s even better is that they readily accept government assistance, making care much more financially accessible.

However, because it operates as part of a larger corporate chain, the environment can sometimes lack that intimate, family-run atmosphere you might prefer.

Pros

  • Offers short-term and long-term care
  • Affiliated with a wider healthcare network
  • Certified for Medicare and Medicaid
  • Provides 24/7 skilled nursing support

Cons

  • Lacks an intimate family-run atmosphere

I enjoyed our visit with our Dad.  He just got here and we are expecting excellent care for him.  The staff have been very kind and informative.   I am hoping that our dad will have more interaction with the residents during meals and activities.  Very grateful that we found this place.

My mother spend the last years of her life at Sunrise. Staff is friendly and attentive. She loved the staff and activities. She loved our visits unannounced and welcoming. She loved the gatherings music and dancing. She enjoyed Easter especially with the community involvement. She liked the ease of transportation to her dialysis and welcomed her family visits. I would highly recommend Sunrise and the friendly knowledgeable staff.

Having access to reliable 24-hour care with proper medical certifications brings massive relief to stressed relatives. San Pedro Manor delivers this through continuous monitoring and helpful government payment options.

When I visited an aging relative here last year, I immediately noticed how much the residents enjoyed the spacious dining room and comfortable air-conditioned lounges.

The property can accommodate up to 150 units, providing ample physical space for new admissions to settle in comfortably.

The inclusion of outdoor areas also definitely improves the daily quality of life for long-term residents.

On the downside, prospective families should carefully review their recent state inspection reports. The facility has faced some health standard complaints in the past.

Pros

  • Certified by Medicare and Medicaid
  • Features comfortable outdoor areas
  • Provides continuous 24/7 monitoring
  • Accommodates up to 150 total units

Cons

  • Received previous health standard complaints

Customer Review

My first time there visiting and everyone was helpful from me signing in to getting to my room where my friend was they call me Dino short for (Dionicio) and they call him Tony short (Dionicio) go figure lol!

I was invited to the Veterans Day BBQ for the residents by Chadni the Activities Director.  One of the residents hugged me and was happy to see me, that made me feels so special.  I went upstairs to second floor where the other Veteran residents were.  It was a joy to see them enjoy their day and their meal.

Finding specialized support for cognitive impairments requires a facility built around both safety and patience, and One Heartland at San Antonio Wellness and Rehabilitation is designed with that in mind.

It operates as a skilled nursing center with a dedicated memory care unit that supports residents with conditions such as Alzheimer’s in a structured environment.

Their trained medical professionals are equipped to manage complex health needs, helping ensure residents receive consistent care and supervision.

Many families choose this type of facility when standard home health care services no longer meet evolving medical needs.

The center also offers comprehensive physical rehabilitation services for patients recovering from major surgeries, making it a more well-rounded care option.

However, the secure design of the memory care wing does mean a more controlled and restricted environment. This may not be ideal for seniors who prefer a high level of independence.

Pros

  • Features a dedicated memory care unit
  • Employs trained medical professionals
  • Offers comprehensive physical rehabilitation
  • Located on a dedicated accessible campus

Cons

  • Creates a restricted physical environment
  • Less suitable for highly independent residents

Wonderful place! Everyone is friendly and there are lots of activities.  They all take really good care of my mother, and she looks forward to spending time there. I’m so thankful that we have SA Wellness for our monthly respite visit.

This is my second time going there I can honestly say is ok facility the only reason I don’t give them 5 stars is because of The Dietary Manager he was rude never got my food right…… But the nurses like Elsa and Rosa are great also I would like to say thank you to miss G she was always very nice and patience because I have strong personality .

4. River City Care Center

Address: 921 Nolan St, San Antonio, TX 78202

Community engagement is incredibly vital for the mental well-being of seniors transitioning into assisted living. That’s why River City Care Center places a massive focus on keeping residents active

They offer regular group art classes and dependable transportation services, which help maintain a sense of routine and participation in the wider community.

The staff is also frequently praised for the consistent, in-person care they provide on a daily basis, which gives families added reassurance.

While some families also explore complementary mental wellness options like hypnotherapy, the center’s structured activities already do a solid job of keeping residents engaged and mentally stimulated.

Residents also appreciate the in-room kitchenettes that offer a small sense of independence.

That said, the facility sits in a highly urbanized downtown area. This means it completely lacks the expansive green spaces commonly found in suburban locations.

Pros

  • Highly rated by community reviewers
  • Offers daily enriching art classes
  • Provides essential transportation services
  • Features comfortable in-room kitchenettes

Cons

  • Located in a highly urbanized area
  • Lacks expansive suburban green spaces

Customer Review

River City Care Center is great!  The staff have been amazing. They were very informative when my sisters and I first called to inquire about a place for our dad. The staff is so kind, caring for our dad and friendly to us. I appreciate the events they host for the residents each month and holiday. If you are looking for a place for an elder parent, I highly recommend River City Care Center. Thank you all for the kindness you have shown our dad and us. God bless the staff!

Came to the facility to see my aunt and was very impressed by all the positive changes in the facility. You can tell new leadership is working on making this the true gem this neighborhood needs. The building is very clean and everyone is very welcoming. The new Activity Director was popping popcorn and showing the residents a retro movie which they were thoroughly enjoying. I can’t wait to watch the place grow!

5. Saint Francis Nursing Home

Address: 630 W. Woodlawn Ave, San Antonio, TX 78212

Contact: 210-736-3177

Moving into a facility that feels spiritually grounding can make a world of difference for religious seniors. Operated by devoted sisters, Saint Francis Nursing Home offers a deeply compassionate and faith-based environment for its residents.

The staff works incredibly hard to keep the entire building spotless and completely free of typical clinical odors. They even serve highly nutritious meals that taste just like comforting home-cooked food, which the residents truly love.

On the practical side, the administrative team is available to help families work through complicated insurance and Medicare paperwork in person, which can take a lot of stress out of the process.

Just note that the heavy presence of Catholic traditions and daily Mass might not align with the preferences of non-religious individuals.

Pros

  • Maintained exceptionally clean and spotless
  • Offers access to daily religious Mass
  • Serves highly nutritious home-cooked meals
  • Provides helpful administrative support

Cons

  • Features a deeply Catholic environment

Customer Review

My mother has been at St. Francis for several years and I could not imagine a better home for her.  The care is excellent and the place is always spotless.  The sisters who run it are always there and she loves the daily Mass.  My mom has made some great friendships, she loves going to activities and she even says the food is, “just like homemade.”  We are so lucky to have this hidden treasure in San Antonio!  If you are looking for a place you will feel truly comfortable leaving your loved one look no further.

My Dad spent his last few years in St Frrancis and they were wonderful. The Nuns that run the facility are a blessing to their residents and their families. The workers could not have be any nicer to my Dad as he slowly faded away. Examples of the place is that it does NOT smell like a nursing home, the food is good, and the other residents are cheery in the building. It shows the love of Christ in every way. It was so nice that when our Aunt needed a place, we waited for a place at St Francis.
